Requesting Record Level Data

Record level information is taken directly from the health records of individuals and unlike aggregate information, is not averaged or grouped. Record level information poses a greater risk to an individual’s privacy and demands greater due diligence before it is disclosed.

Record level data can be requested for projects requiring ethical review (research) and those that do not require ethical review (evaluation). To determine if your project requires ethical review you should contact the Health Research Ethics Authority (HREA) located here

To request record level data for projects requiring ethical review you will need to complete the Common Application From for Research found here

To request record level data for projects not requiring ethical review you will need to complete the Common Application From for Research found here
